Anti-vascular endothelial growth factor therapy has revolutionised the treatment of a variety of ophthalmic conditions and has become the first-line therapy for a range of retinal diseases. Bevacizumab, a VEGF inhibitor first approved for the treatment of colorectal cancer, has been shown to be nearly or virtually as effective and safe as other anti-VEGF therapies in the treatment of certain retinal diseases but is not approved or registered by the Food and Drug Administration or European Medicines Agency. While other anti-VEGF (...) options are approved and registered, they are generally more expensive and less accessible. Accordingly, despite its off-label status, bevacizumab is frequently used for a variety of disabling retinal diseases. Indeed, bevacizumab is included on the World Health Organization’s list of essential medicines. However, its use in some parts of the world remains restricted due to its off-label status. How, then, should healthcare authorities approach this situation? What are the ethical and societal implications of adhering to a standard and generally effective evaluation and approval system while restricting access to a potentially cost-saving therapy? In countries where its use is not restricted, how should providers approach off-label treatment with bevacizumab? By examining the evidence behind bevacizumab’s efficacy and safety and evaluating the individual and societal implications of off-label use and restriction, this paper illustrates the ethical factors providers and policy makers must consider in the off-label use of bevacizumab and ultimately argues for increased access to bevacizumab in the treatment of retinal disease. The pragmatic character of the Dutch book argument makes it unsuitable as an "epistemic" justification for the fundamental probabilist dogma that rational partial beliefs must conform to the axioms of probability. To secure an appropriately epistemic justification for this conclusion, one must explain what it means for a system of partial beliefs to accurately represent the state of the world, and then show that partial beliefs that violate the laws of probability are invariably less accurate than they could be otherwise. The first task can be accomplished once we realize that the accuracy of systems of partial beliefs can be measured on a gradational scale that satisfies a small set of formal constraints, each of which has a sound epistemic motivation. When accuracy is measured in this way it can be shown that any system of degrees of belief that violates the axioms of probability can be replaced by an alternative system that obeys the axioms and yet is more accurate in every possible world. Since epistemically rational agents must strive to hold accurate beliefs, this establishes conformity with the axioms of probability as a norm of epistemic rationality whatever its prudential merits or defects might be. In this paper I examine how the constituent elements of a firm's organizational structure affect the ethical behavior of workers. The formal features of organizations I examine are the compensation practices, performance and evaluation systems, and decision-making assignments. I argue that the formal organizational structure, which is distinguished from corporate culture, is necessary, though not sufficient, in solving ethical problems within firms. At best the formal structure should not undermine the ethical actions of workers. When combined with a strong culture, (...) however, the organizational structure may be sufficient in promoting ethical conduct. While helpful, ethics training and corporate codes are neither necessary nor sufficient in promoting ethical behavior within firms. (shrink)

In The Logic of Decision Richard Jeffrey defends a version of expected utility theory that advises agents to choose acts with an eye to securing evidence for thinking that desirable results will ensue. Proponents of "causal" decision theory have argued that Jeffrey's account is inadequate because it fails to properly discriminate the causal features of acts from their merely evidential properties. Jeffrey's approach has also been criticized on the grounds that it makes it impossible to extract a unique probability/utility representation (...) from a sufficiently rich system of preferences (given a zero and unit for measuring utility). The existence of these problems should not blind us to the fact that Jeffrey's system has advantages that no other decision theory can match: it can be underwritten by a particularly compelling representation theorem proved by Ethan Bolker; and it has a property called partition invariance that every reasonable theory of rational choice must possess. I shall argue that the non-uniqueness problem can be finessed, and that it is impossible to adequately formulate causal decision theory, or any other, without using Jeffrey's theory as one's basic analysis of rational desire. A distinction should be made betweentwo types of ethical problems. A Type I ethicalproblem is one in which there is no consensusas to what is ethical. A Type II ethicalproblem is one in which there is a consensus asto what is ethical, but incentives exist forindividuals to behave unethically. Type Iethical problems are resolved by making,challenging, and reasoning through moralarguments. Type II ethical problems areresolved by changing the institutionalenvironment so that people do not haveincentives to behave unethically. Type Isolutions, however, (...) will not be effective insolving Type II problems. This study examines the self-reported ethics of both current and future advertising practitioners, and compares their responses to four scenarios and 17 statements on advertising ethics. Stepwise discriminant analysis was used to determine the extent to which both groups applied the classical ethical theory of deontology to the scenarios and statements. Results indicate significant differences between both groups. For example, current advertising practitioners are significantly less likely than future practitioners to apply deontology to decision making. This paper is a response to the preceding papers. It is maintained that American business is failing to live up to its obligations to society. One reason for this is acceptance of what De George calls the Myth of Amoral Business. Businessmen believe that morality is either not applicable to business or that business has a special morality of its own. Several arguments are advanced to show why this is not true. A second reason business is failing to fulfill its (...) obligations is that egoism seems to be an essential feature of capitalism. Harrington believes that this should be changed, but that attempts to change it are bringing about a more collectivist society. Kirk sees no need for change because he believes capitalism the best possible economic system. It is argued in reply that they are both mistaken because their analyses are based on outmoded ideologies which impede rather than aid us in solving problems caused by the irresponsibility of business. This article introduces and discusses a series of problems which any adequate account of legitimate practical authority must be able to solve. I then argue that Joseph Raz's influential Service Conception of Authority is unable to solve them. I develop a new account of legitimate authority by integrating many of the important insights of the Service Conception into my own framework for understanding the nature of moral rights and duties. John McDermott, Streams of Experience I investigate one manner in which classical American pragmatism might be utilized by theorists and practitioners interested in addressing urban environmental problems. Despite the widespread adoption of the sustainability moniker within the environmental movement, (...) evidence suggests that progress toward implementing urban environmental sustain ability proposals has been minimal. To address this inaction, I undertake an analysis of the philosophy of progress guiding efforts to transition urban environments toward sustainability. I argue that one of the reasons so little has been accomplished in terms of implementing existing urban environmental sustainability proposals is that a disproportionate emphasis has been placed on values that stem from economic-centered indicators of progress. I argue that the value of progress ought to be less about how much of a certain type of economic growth sustainability proposals ultimately can generate for urban environments and more about ensuring that continual incremental societal progress takes place. Over half of the world's population (3 billon people) now lives in urban environments. The combination of people, industry, and commerce enmeshed in environments over-determined by plans, designs, and configurations that continue to emphasize ease, efficiency, and spatial sprawl over ecological constraints and sustainability help to make urban environments the primary contributors to multiple types of ecological degradation. With this in mind, urban environments demand greater sustained theoretical and practical attention than has been and is the norm under status quo (...) approaches to urbanism that pay little regard to ecological constraints and under traditional models of environmentalism informed by an anti-urban bias. Fortunately, an emerging form of urban environmentalism exists that recognizes the central ecological role of urban environments. This emerging form of urban environmentalism also recognizes urban environments to be the places where environmental problems are often most severe and where environmental solutions are in most need of immediate implementation. The upshot of this dual recognition is that it is accompanied by a growing dual awareness of urban environmental problems and solutions. In this paper I argue that despite this dual recognition and awareness, a paradoxical form of inaction is the norm when it comes to implementing urban environmental solutions. I explain why this is the case by outlining what I call the paradox of urban environmentalism. I explain why the paradox should be considered one of the most significant problems of this new environmental century as well as that frame which opens the door to a new sense of what is possible in urban environments. In the process of considering what is possible, I arrive at some preliminary conclusions about a possible methodological role philosophers can play in helping to address this paradoxical inaction in the hopes of bringing about more sustainable urban environments. While individuals and movements openly advocating “Buddhist socialism” only begin to appear in Japan in the first decade of the twentieth century, germs of the idea can be traced back to the writings of a few scholars and social activists of the 1880s. One example of the latter is the Eastern Socialist Party, founded by TARUI Tōkichi 樽井藤吉 in 1882. Though the party was short-lived – setting a dubious precedent for left-wing parties over the next 50 years in being forcibly (...) suppressed by the government within months of its inception – the writings of Tarui and other founding members were, for their day, quite radical, and provide an early example of the tension involved in attempting to transform the world – “make it new!” to use the modernist catchphrase – while remaining true to one’s cultural roots. The “draft of the party’s regulations”, written by Tarui, contains 17 articles, along with a number of sub-clauses. Here, we see several of the tensions that would haunt Japanese experiments in progressive and radical Buddhism over the next several decades. First is the natural but difficult attempt to “indigenize” socialism by appealing to traditional Asian concepts and ideas; second is the appeal to the East Asian values of peace and harmony, which was frequently accompanied, among early socialists, with an appeal to the Emperor as benevolent protector of the social welfare of the Japanese people.
